Incoming Student Prerequisites
- Thomas More College, in accordance with the recommendations from the American College Health Association and the Centers for Disease Control, recommends that all incoming students submit to the Campus Health and Wellness Center records of their immunization history, and complete a health history form. All students born after 1956 should have two doses of live measles vaccine (MMR). In addition, each student should have at least one DPT/DTAP or DT, and one Polio after their fourth birthday along with a Tetanus booster within the last 10 years. The Hepatitis B series and the Meningococcal Vaccines are also highly recommended. Students residing on campus and International students are required to submit vaccine records prior to arriving on campus. Any student who has not submitted immunization records will not be permitted to check in the residence halls.
Proof of immunizations should be presented to the Campus Health Center on registration day. You can obtain this information through your physician’s office and mail them directly to Thomas More Campus Nurse or fax them to 859-344-3636.

- An International Health/Immunization Form is required and should be sent to the firstname.lastname@example.org prior to registration day, in addition to the other 2 forms mentioned above, which should be brought with you to registration day.
